Registration

You can use the enclosed Registration Form to register for the Symposium and all events as well as for hotel reservation. Please return the form as soon as possible by mail or fax to the Symposium Secretariat (Congress & Hobby Service P.O. Box 1022., H-6701 Szeged, Hungary; Fax No.: +36-62-484-532). You can also register via Internet for the Training Course: www.congress-service.hu/2012/danube.

Registration fees for participants includes:

• Admission to all scientific sessions
• Admission to the poster- and trade exhibition
• Congress material (final program etc.)

The registration fee for accompanying persons includes:

• Admission to the Welcome Reception on 7^th June 2012
• Sightseeing tour in Szeged

Optional Program:

Fishermen’s party on 8^th June 2012

Near Szeged an authentic Hungarian menu (including the famous Szeged fishsoup) will be served for the guests.

Price: 35 EUR with transfer.

Lunch (7th and 9th June)

At the venue of the Symposium (SZTE TIK Congress Center). Price: 16 EUR.

Payment

When registering for the 44^th International Danube Neurology Symposium please arrange payment of the registration fee. All payments should be made in EURO.

Any bank charges will be charged to the payer.

In case of bank transfer by your sponsor / workplace please make sure that your name is forwarded together with the money. If your sponsor uses a block transfer for several participants it is necessary to include all relevant names.

Confirmation

Upon receipt of your registration and payment, the congress office will send you a confirmation form that also serves as your invoice. Please show this confirmation of registration at the congress counter when picking up your congress materials.

Registration will be confirmed if payment is received by the Congress Office by 31 May 2012. Should payments be received after this date, you will receive your confirmation at the Congress Counter in Szeged. Please bring in this case a copy of your bank remittance with you.

Passports and Visas

Currently valid passport or identity card is required for foreign visitors travelling to Hungary.

Although visa requirements may vary from country to country, no visa is needed from almost all

European Countries.

Liability - Insurance

The participants take part in all excursions at their own risk. The registration fees does not include any kind of insurance. The Organizing Committee and the Congress Bureau can not accept liability for personal accidents, lossess or demage. Health, baggage or accident insurance is recommended and must be taken out in your own country.

Cancellation of Congress Participation

In the case that you cancel your participation at the congress by 5 May 2012, the registration fee minus a processing fee of 50 EUR will be refunded. No refund will be made if cancellation is made at a later date. Please notify the conference office of your cancellation in writing.

Modification of Congress Participation

In the case of cancellation or modification of any of the datas of your invoice, it will mean a handling charge of 15 EUR.
